It depends on your usage, but if you're finding you need more pressure or that the nib is dragging along the surface, it's a good indicator that new ones are needed.

And random advice is random. I've got wrist issues, shooting pain, numbness, weakness and I used to have to spend a lot of time in braces. Which really sucks for drawing. But you know what really helped me out?
Guitar Hero.
Seriously. I started playing Guitar Hero for a couple of hours a week and I haven't had to wear my wrist brace since 2009.
I don't know if it's your wrist or not, but if so, get thee to a game store :)
