Substitute a condom for the fortune and you've recreated a practical joke that a girlfriend played on me. Unfortunately, grilled cheese is kind of stretchy and I hardly chew my food so if I hadn't been told it would have been a couple of days before I found out.

He's just confused because "Beartato" is a common nickname for people named "Randall".

It's kind of a thing on XKCD. People started using it when Randal posted a comic with a reference to something they were thinking about. This was in line with one common understanding of the idiom which is a suggestion that someone has read your mind. (Leaving aside another context in which one believes that someone is affecting, controlling or invading one's mind ( ... )
