My massage therapist says TMJ is caused at least in part by a mis-adjustment elsewhere in your body that is throwing you out of alignment. Since your vision wants to be level with the horizon, this will also throw off your neck/jaw. So watch your posture.

what are you eating? are you chewing gum? one of the first things my dentist told me was to eliminate gum and crunchy foods. i do notice a big difference when i cut those things out (she said around a huge wad of gum.....i caved today) though it sounds like you might have a worse case than me and that might not help. in any case, i hope you find relief!
I chew gum like a maniac. But whenever I'm having TMJ issues (it's intermittent for me, no clue how common that is) I have to stop and it makes me insane. But at least the pain stops.

Also massage from a skilled trained therapist who knows what they are doing.
(I happen to do a lot of TMJ massage - however I'm in Albuquerque).
