You're the third person in my life who's hired a personal trainer this year and experienced really positive results. Good for you! The thought scares the shit out of me. I mean, that's like committment and stuff! But maybe it's something I should start considering.

When you lose weight via proper exercise (Balanced effort between weight training and cardio) you tend to shrink without losing weight at first:

You replace fat with muscle... fat is less dense than muscle, so it takes up more room. You're not losing weight, you're gaining tone and strength. (Both good, btw.)

Then, the magic of biology begins to take over. Muscle tissue requires more calories to stay alive than fat tissue. If your diet stays the same, your body will require some more energy to stay powered up, so it'll dip into your reserves: fat.

To be honest, your surgical option may not be needed with a good training system. Get your core (central torso muscle... abs, back, chest, shoulders) stronger, lose some excess weight, and things should get better. As well, with some weight loss, your bewbies won't be so big - they'll go later in the process, but it's fat that makes 'em so soft and fun to have around. Lose fat, and they'll shrink some.
