With the summer heat, I'm sure many muslims and non-muslims are wondering when it might be appropriate to break the fast.  This article answers the question clearly and succinctly:

So it's essentially the same rules, then.
The sickness rule would cover things like brittle diabetes, where fasting could cause someone to pass out, but someone whose blood sugars are pretty well controlled could fast without fear of serious health consequences.
Theoretically someone who was otherwise healthy but whose job was an outside job in heat like we're having right now, where to go without water could lead to dehydration, heat exhaustion, heat stroke, and possibly death.

thank you. so far i've been blessed. i had to break the fast today to prepare for a medical procedure which required i take meds. but i'm keeping to the fast for the rest of the day even though i plan to make it up. islam doesn't promote asceticism. so it would be in keeping with that mindset that all observances not be made if they could bring harm. also, those who cannot fast because of illness have the alternative to pay zakat (similar to charity) in the amt of what it would take to feed one person for one day. i think it's been in the $10.00 the last few years.
