As for how loyal Sam was to Frodo? Based on the expectations of what a manservant is like, if I remember correctly; the loyal retainer types who practically live for their masters and little else. Just in Sam's case, Frodo treated him kindly and on equal footing with himself.
I forget where I read that explaination, but it was in one of the various 'explaining all the behind the scenes things about LoTR' articles.
(And I need coffee because that whole sentence structure sucks and I don't have enough active braincells yet to fix it. >.<)
