I'm imagining a story where the Daddy/Baby relationship is both parental as well as sexual, including the usual aspects of having a baby (diapers, bottle feedings...etc...but "Baby" sleeps in "Daddy's" bed, not a crib).
Sex between Daddy and Baby is con or dub-con, but non-con isn't a deal breaker so long as its not overly violent or cruel. Bottles are usually milk or juice but for a special treat its filled with Daddy's pee (Baby can also drink from the source as well).
Mpreg is the Baby having a baby and trying to understand and adjust to the situiation. (Up to anon if Jen is just really good at pretending or can somehow put himself into the mentality of a baby/toddler.
The scene stuck in my head is Daddy and Baby having sex/making love and Baby has his special bottle (or a pacifier) the whole time.

Warnings: A/B/O mostly in the background, watersports mentioned but really only tangentially (sorry?), very vaguely described sex, sexualized infantilism, and some vomiting and a little medical talk.
Jared rocked slowly in the rocking chair, both his babies held safe and secure on his lap as the motion of the chair facilitated his gentle, lazy thrusts into his first baby, Jensen. Doctor Jim Beaver had been Jensen's doctor for years and stayed with him during and after Jared and Jensen's transition into infantilism. He knew Jensen almost as well as Jared did, so he knew the doctor would leave no stone unturned in his diagnosis.
Jensen didn't have a fever, any kind of rash, or any other symptoms helpful to narrowing things down. He frowned slightly as he palpated Jensen's abdomen before switching to a grin and giving a little tickle.
"I'd like to take a little blood sample and talk to your Daddy," Doctor Jim addressed both Jared and Jensen, "but then I've got two lollipops for you--one for now and one for later, okay ( ... )

"That was Doctor Jim, Baby, and you'll never guess what he said!" Jensen cocked his head to the side and waited for Jared to continue. He was sure the reaction was going to be priceless. "We're having a baby, Baby!"
Jensen's jaw dropped comically, his pacifier falling to the ground, and Jared absently noted that it would need cleaned properly before he gave it back to him. Jen's shocked expression remained for a few more long seconds as the news sunk in and then he grinned the brightest, most beautiful smile Jared had ever seen, both his hands automatically moving to his tummy. He looked down at it and when he looked back up, his expression was one Jared hadn't seen in almost two years.
"We're having a baby, Jay!" His voice was husky from disuse, and Jared had almost forgotten how much he loved the sound of it. Baby Jensen didn't speak much. Jensen's expression changed to a concerned frown. "A real baby. We won't be able to go on like this ( ... )

Awesome! I'm even okay with the vague "special bottle" mention since you pretty much hit everything else. The history of how they got into it was orginal and believable...well, as much as mpreg can be anyway. :-)
And you added Jim Beaver, which is another bonus I never thought to mention. I love how he was so supportive of them.
