As for Britishisms - I now point out how terribly old I am by sayinh that nurses usually didn't wear white uniforms in the late 60s - a staff nurse would wear pale blue with a white pinafore in most hospitals. When I started my training in the mid 70s we were some of the first to wear white dresses.
Also - we would not have used the phrase 'attending physician' (we still wouldn't) - we would wonder what he was attending... We would usually just call the doctor on duty in A&E 'the registrar', or 'the medical registrar' if he was a physician rather than a surgical reg. In a smaller hospital where the Casualty Registrar only worked days we would have had an 'on-call registrar'. It was often the job of a junior nurse to go and hammer on his rest-room door to wake him up if we needed him, I recall!
