Esme clicked her phone off, looking around. Listening first to see if Carlisle was working, while looking for her children. All, but especially the three currently away.

Ed, as a matter of courtesy, stubs out his cigarette when he spots the woman entering.
She gets a hopefully sympathetic smile. Being lost and finding this place...well, can be pretty damn lucky but she doesn't look like she's on the run from everything.
Smiles are meant to prevent things from being disorienting.

She isn't new or uncomfortable specifically with the people in general, so Esme returned the smile, easily, once she made sure the basket was balanced properly on the table.
