If you asked Otto, he might reply that as with anyone, it varies from individual to individual, and anyway Villiam stereotypes are insulting, nobody can possibly be expected to be alert all the time.
Which is to say: He doesn't see her right away.
When he does, however, he smiles a startled, pointy smile and sits up, glasses slipping slightly.
Upon reflection, Otto is not surprised that the Lady Margolotta has found her way to this place. In fact, the only thing that would surprise him less would be if the Patrician has as well.
"The feeling is entirely mutual, dear lady, I assure you." He marks his page with a bookmark and lets the tome fall closed. "Have you known of this place lonk?"

He is indeed wearing a lot of black, and all of it a hell of a lot (no pun intended) messier than Otto's. He's also licking a bright pink ice cream.
