When Doc stumbles downstairs after the third nightmare, something is definitely wrong compared to the other two he's had before. There are no rope burns. There are no real bruises.
He's shirtless, and carrying a rusted, bloody scalpel in one hand. His chest has been carved with a blade to form a 'Y' shaped wound, from shoulder to sternum to just above the belly button. Not all the way into the body cavity, but deep enough to where the blade nicked bone.
There are ether soaked bandages packing one wound on the shoulder, the skin raw and red.
Another thin slash across his throat (just a surface wound, nothing fatal) but it's there.

Infirmary. He's stumbling and trying not to fall but he's so tired. So very tired.
Most noticeable about the familiar face that stumbles downstairs is the fact that someone has taken a pair of scissors to his head. His once-blonde hair is now streaked with blood and chopped in a quick cut to make it short, pieces of every length.
