in a few easy steps
First select the picture that you want to use for your base. I picked this on. When I cropped it, I cropped it so that the height is 100, the width, you don’t want to be 100.. Mine was 100x83
I duplicated it twice, setting the layer above the base to Screen 58% and the layer above that to Soft Light 100% Sharpen if needed.

Now I duplicated the base again and brought it up to the top, set it to Soft Light at 100% and desaturated it.
I duplicate it again setting it to Screen at 40% and desaturate
Duplicate again and set it to Multiply at 85% and also desaturate.
Merge.
Now make a new 100x100 file with a white background and paste you picture on it.

I decided that my picture took up too much of the white base so I took the rectangular marquee tool and cut off the right side of my picture.

I take this brush from Bruno’s Honeypot Vintage Grunge collection
http://www.ancient-secret.de/brushes.html In black I make a line along the edge of the picture.
Next I make a new layer and add my text. I wrote ‘gunslinger’ in Print Clearly at 22pt. I rotated it 90 degrees CW and placed it along the right edge of the picture. I duplicate that layer.
Next I took the brush that I used earlier and made a line above my text.
Then above that I just added some tiny text and I was all done :)
