He never really bothers to see who's at the door before he opens it anymore--anyone who would want to get in for nefarious reasons is going to get in whether he wants them to or not, because most of the people who want to hurt him have abilities that render locks and chains pointless, and resisting will just piss them off more and put him out the cost of a new door.
But resignation on that score doesn't make him any less frantic or upset to see Sylar on his doorstep--carrying a body, no less.
"Sylar! Why are you here?" Matt and Molly could be home any second, and Molly shouldn't have to see any of this. Mohinder fights the mad urge to slam the door on Sylar's hand.

The fact that Sam has a nice large bullet wound to the head might not help Sylar's demand any.

Ah. The location of the wound explains it all.
"I'm not going to remove that bullet for you just so you can have his brain," Mohinder snaps. "And--there's no hope for him anyway. Just look at him. I can't do anything about a bullet to the head."

When he doesn't find what he'd apparently been searching for, he turns back to Mohinder. "This isn't just anyone, Mohinder. I don't expect you to do anything for him. You can't help Him, you're right about that. You are going to be fixing me." His hand hangs next to the large glass vase on the table next to the couch. Just in case.
