I went for a free evaluation at Anderson and Shapiro on the far west side (near Old Sauk). They're the people who did Kara's implanted lenses and I've heard great things about them from everywhere. They flashed a lot of lights at me to determine if I was a candidate (I am) and discussed the possibility.
When I got evaluated about a year ago it was about $2,000 per eyeball for standard Lasik.
Yup, my eyes are pretty damn bad and funky but apparently they can correct quite a lot.
I don't have a discount program but with my HSA it comes out to something like $140 per paycheck. I'm not doing it in 2009 but it's in my plans for 2010.
I didn't get it done in Madison, but I did go through the TLC franchise. My surgery was excellent, and the middle package that I went with came in under $3500 total. I'm definitely very satisfied.

And, at the risk of sounding like an infomercial, my life is so much better now. I don't know the exact numbers, but my eyesight used to be pretty horrible - I could see the big E, and maaaybe the line under that, but that was pretty much it. Now it's 20/15 - better than perfect!
Of course, it's different for everyone, and I'm sure you've read up on the risks, and wherever you get it done will tell you about them all over again, but besides that, I recommend TLC specifically [although I'm sure the other recommended places are fine, as well!] and the procedure in general.

2k per eye? I think with my weird vision insurance/discount program + HSA it may actually be worth it.
