grammar question!

I'm not sure if the following use of "all too familiar" should be hyphenated or not, and Google is less than helpful. Your thoughts?

"Her eyes narrow in an all too familiar obstinate expression"

I'm cracking open my grammar book ; here goes :
half hour's OR half-hour
nine-hole : hyphenated

all too familiar
no hyphenation for simple verb phrases: She is good at decision making.
hyphen when the phrase modifies a noun: decision-making process

"all too familiar" = never hyphenated
"nine-hole course" = hyphenated
