I've been jumping around various sale sites and have seen a lot of things on the sales comm labeled "plus sized" that I'm just like "wha- I can't even....." my bust is a solid 42 inches or 104/5 cm (bra sized D) and the item I see is like 30 inch bust. What measurements are considered plus sized by brand and what do you guys consider plus sized in

Many brands offer some pieces which allow for a wider variety of sizing. Angelic Pretty and Metamorphose are the two best brands for ladies with larger busts, as they often include lots of shirring.
If you do a search of daily-lolita using their plus sized tag (http://daily-lolita.livejournal.com/tag/plus sized), it should help you out with some ideas of what will fit.
My situation is pretty similar to yours (large bust, smaller waist), but I've found that AP jumpers with full back shirring are very forgiving and don't look too bad when worn.
