(1) Which was admittedly old news to fans of the source comics, and I rather suspect this character's identity of being a throwaway decoy spoiler.

I know that isn't dangerous, but he himself is a danger. For the record, the character was a brainwashed assassin who had no real idea what he was doing. And thanks for the message, I may do that :)
I've been pondering this and I can't see any way to brainwash a multiple. The reason a person becomes divided is that something(s) were so terrible the core personality shattered. Not all thee personalities know each other. More important, this group of personalities are built with one directive - protect the person so that all the personalities are protected. At least some of the personalities would be aware of the brainwashing.
Have you ever read 'when Rabbit Howls? It was written by a woman with DID, and it gives you an idea of how personalities are organized.
Errm, well actually it would be something the character developed later. Like the process sort of splintered his mind a bit. I don't know if you've seen the second Captain America movie or not. But still, thank you for the information.
EDIT: Basically it's his old personality competing with the personality he developed when he'd been brainwashed and the two personalities overlapping. I'm not even sure that would count as DID. It's just the closest thing I could find to describe it.
[Spoiler (click to open)]Bucky as portrayed in the movie is probably going to be nigh-uncontrollable by hospital or EMS staff unless he submits to care. I mean, even with a dislocated shoulder he manages to [Spoiler (click to open)]swim after Steve and drag him ashore and there's probably no one without superhuman strength capable of fighting him to a standstill and disabling that prosthetic arm. It's debatable whether the arm can be removed (in comics canon it could be, but I doubt so in movieverse) and if they can't disable it, he could tear his way out of pretty much any lockup. I mean, the guy can punch through asphalt like it's plasterboard. Handcuffs, doors, bars, fences, none of it's going to stand a chance unless he wants to stay wherever they put him.
I think maybe to circumvent this, the author could specify that it's a hospital that specializes in treating supers? they could have other supers on staff to help with containment, the building would be built to sustain damage, and the other patients would be able to "hold their own" if one of them gets violent.
WHAT other supers? In MCU canon, there are a handful of known people with superpowers, most of whom were in SHIELD custody or on Asgard. Even in 616, there's no hospitals specifically for supers -- remember when Luke Cage and Jessica Jones were turned away from the hospital when they were having their baby, because of the doctor's fear of their powers running amok? Dr. Strange (who's a heart surgeon, not an OBGYN) had to deliver Luke and Jessica's baby. If there's no hospitals in 616 dedicated to caring for superpowered patients, there sure as hell aren't any in MCU.
The Winter Soldier simply isn't containable by civilians unless he submits to care.
perhaps it's not canon, but is the winter soldier being checked into a psych hospital canon? no. but the writer is gonna write it anyway because it's fanfic and they can do what they want...which could also include altering canon to include one made-for-supers psych ward existing in their fic. it's really not that far outside of the realm of possibilities, geez.
BTW, I imagine he'd be considered a "John Doe" at first, but I hope someone can tell us what would happen if medical staff established his real identity? [Spoiler (click to open)]He would most likely have a birth certificate on file with the state of New York, but one from 1918. Who would even be considered next-of-kin at that point? Are they gonna dig up some great-nephew or niece?
