Unless your character is stubborn to the point of insanity, there's no way he'd try to repair his own rectal stitches. Not only would he not be able to see the area (without a mirror and some contortion), he also would have trouble manipulating the needle and thread with his hands behind his back like that. He might start, but my guess is he'd give up before finishing the first stitch.
Try tying a bow knot behind your butt, then figure it's at least twice as difficult as that plus pain. I won't say impossible... but pretty close.
