Don't know if it helps, but I had a tonsillectomy in 1962 and definitely had to do the countdown from 20. (And again when I had adenoids removed in 1969; I have no idea if that was specific to children, though.)

As for additional intravenous anesthesia, this article from 1955 seems to say (after a short skim) that it was more or less still in the experimental phase at the time; IIRC, the usual procedure was to put a mask over the patient's mouth and increase/lower the gas flow as needed during the operation.
