Ear infection, some dental issues. You're looking for things where the pressure change is going to create problems.

I can tell you that flying with an inner ear infection is very painful, as is flying with a sinus infection. I actually passed out from the pain from the inner ear infection.

Like everyone else, an ear infection would be the best bet. I had to cancel a flight because of it once. The doctor said something along the lines of my ear drums bursting. Hopefully this helps!

A broken hip would make it very uncomfortable to fly, unless they were in first class/could recline while flying.

Any back injury, even a bad strain, might make it extremely uncomfortable for her to sit in a small seat for that amount of time. Of course this would also apply to a car if she's the one driving - if not she could recline the seat or lie down in the back.

Also, it would be easier to stop and stretch/ walk/ whatever would help the pain. But still not great either way, for sure...
