I mean, they had the love but they also had the repeat-bickering thing come around regularly.

Given the quote about 'sex with his mother' as a fantasy (dream?) I'd say that the only difference between John and most people was the lack of a filter for the random that most people don't say out loud-- especially when it's fleeting.

Personally I think that for some relationships people are so close and they only way they can think of to describe or devine that is through sexual relationships.
