I really love this story. The characters are terrific and the progression of the relationships are perfect.
I'm not sure that adding Sam would take away from the twin thing. It's not going to change what/who they are and that they have a bond that Sam doesn't - namely the psychic thing. Adding Sam to their sexual relationship wouldn't take away from that but it would strengthen the brotherly bond as it seems that Sam sometimes feels like an outsider. Almost as if it's Dean/Priestly and Sam and not the three working together.
I think you can also. If you had made his relationship with them strictly brotherly in the normal sense, maybe not but you can see the feelings are there between them all. Especially when Priestly had Sam get in bed with them and Priestly was petting Sam's hair.
Plus, you aren't doing it for the sheer shock value. You are doing it as a progression of their relationship. The background is there for it.

I've really been turning it over in my head, and I think I can introduce Sam into it without derailing the plot.
