How would being adopted affect one's security clearance? I'm curious!
I know that withholding this information could have serious repercussions when one's medical history must be known.

When a person applies for a high level of security clearance extensive background checks have to be performed, including natural parents. They want to find out where the parents are from and who/what they may have been involved with. There are circumstances when a parent's past could influence the child. If the child is unaware that they're adopted they will claim the adoptive parents are their natural parents and get their clearance. But if at a later date they discover who their real parents are these unknowns could exert influence and pose a security threat. Unlikely but possible.

I don't think there's an obligation at all as far as I know.

I wouldn't be surprised if this situation has come up in the past, and the services have contacted one or other set of parents and asked them to fess up rather than doing it themselves.

I should add, this is from personal experience of only finding out later on that the wolves were my adoptive parents, and I was actually born a badger.

Must suck to find out you're a badger and not a wolf. You must have been, like, the slow, retarded brother alongside you're adoptive siblings.

Have you ever seen the sheer destructive power of a badger? Tear one of those noncey wolves apart in seconds, a badger will.

Just imagine the inbreeding potential if the adopted children are not told.
