I think Yohji used Ada's key to change her and she freed Wesker and killed him.

She mentioned Yohji fussing when ...Yes, and he'd seemed rather unenthused with the result. And then she disappeared. By the time she returned, she had changed back, but...

She told Angeal she'd guard Wesker that night, Wesker ended up freed, Yohji ended up dead.

We don't really know the extent of what those keys do. What if Yohji was trying to change her back but it... Went too far?
