On second search, I'm also seeing examples where TITLE= and PERFORMER= are used prior to the first FILE= line. It's safe to assume that those write the title and author of the entire CD.
I'm also getting a "read error" (which doesn't seem to harm anything if I ignore it) prior to starting a CD-Copy of my burned audio CD. It's curious, and I want to see if I can find out why it's occurring.
Note the exact 2.000 second space between index 0 and index 1. this is what allows the CD player to continue to play while changing tracks. (track 1 does not require an index 0)
I don't think this is exactly the case. Digging even further pulls up a list of cuesheet commands and programs that support them, but it doesn't say whether it's an exhaustive list. This site claims (perhaps more authoritatively) that an additional INDEX 00 is placed before INDEX 01 to create negative space on the track, so that skipping directly to the track itself will skip the

Have you checked out the burning capabilities of EAC? I finally installed it on the system with the burner on it. It will drop subtle hints when the cue sheet is messed up, and with a little trial and error, it will teach you what they're supposed to look like. I can drag an existing .CUE into it to make it go, or I can have it generate one for a CD it's currently looking at.
If it does something wrong I'll drop another note, but it looks good.
