crop, resize to 100x100, sharpen once then smooth out the skin using the blur tool if needed.
My way of getting the B/W is a little different than most. Image -> split channel -> Split to RBG. That will give you 3 different b/w images, now just choose which ones you like best, for this I chose the green one. Copy that and paste that onto your base.
take this texture by wash_when_dirty lower the opacity right down, erase the parts covering the person in your icon. my settings
until you get this
resize to 80x80
add a 1px boarder in white
add a 9px boarder in black, then merge if adding the boarders hasn't already done that for you.
then duplicate the base, sharpen the top layer once, reduce the opacity til you are happy with it keep the layer setting set to normal. And you're done!
