Most of the time I don't notice much in the way of lag problems. Last night was quite laggy, but otherwise it's been great. Of course, I am on a cable modem, which helps.
Populated? Depends on the city. Some are definitely more active than others. I've sampled East Jerome and Interhogan, but my home base of Blazing Falls is by far the best. Lots of people, and for the most part they are friendly are reasonably polite. ;)
