It's possible for a neck manipulation to release a blood clot to the brain and cause a stroke. It's not very likely.

If you have a somewhat shoddy chiro, you could have them manipulate a fractured vertebrae and break it or release bone fragments into the spinal cord. (my idea would be brain stem, right at the atlas or C1). It's harder to catch fractures on x-rays, but some chiros don't do x-rays unless there's major immediate trauma (falling on your head, car wreck, etc).

Also, the deal with the neck adjustments is that they can feel kind of violent and twist (depending on the technique/chiro), so the layperson is often afraid of getting their neck broken or the like.
