he is not old, he is juts deformed, he was one of orochimarus prisoners, or testsubjects.
He saved her in the ending, when Guren was in the water.

Orochimaru x Sakura is so underappreciated a crack, it just craves for some twisted fanarts ^^

The man, as dear anon has mentioned above, is one of Oro's test subjects with a very Quasimodo personality (if we believe Guren was some kind of Esmeralda)).
That very last moment, when he saved her from crashing into crystal dust, oh...
It brought tears to my eyes, which was a remarkable occasion :)
