This specific example is condemned as "selfish" by closed-minded parents who can't imagine anyone *gasp!* having a different life from what they have and/or seeing anyone else's decisions as a judgement on their own

That, more often than not, really isn't an "and/or" situation. The majority of the people who have a problem with anyone who doesn't see things their way translate the disagreement into a judgement on them. And it's very, very hard to change their minds.

Yes, that's true, but I tend to use and/or to cover the 1:100000000000000000 chance of there being a case that actually IS or, not and. Though the difference usually isn't discernable.

Publishing a book online: I'm reminded of how fanfics are done at Twisting the Hellmouth (http://www.tthfanfic.org/) Chapters and navigation is pretty good, and there are options for all of a story in one shot that make sense. There is export options to epub and kindle.
