I was really shocked at the ending..
So the real truth is that Jae was the one who died?
Then how come there is this part where Jae woke up in the hospital, he was told Junsu died?

Yes, Jaejoong died, and the part about where he woke up in the hospital, it's all in his imagination. He's like a ghost who didn't take the path to heaven, haunting the places he used to visit with Junsu, but in his own dimension. Only at the end he's granted the permission to be in the dimension of the living because it's his and Junsu's anniversary.
