You're right, the journal format does require some suspension of disbelief, but I was so in love with the book as a whole that I had no trouble going along with it. The relationship between Will and Dr Warthrop IS messy and complicated and sometimes infuriating, but I think this will be addressed more explicitly as the series progresses (I've read the second book and there are hints in that direction; hopefully there will be even more in book 3).

I'm glad to hear that the relationship between Warthrop and Will Henry is further addressed in book two, even if it's just a bit. I liked that it was layered and messy and it did reflect Warthrop's personality aptly (I mean seriously, how could a guy like that be good with kids?) but it really did break my heart. I do own the second book and can't wait to get around to reading it. :)
