[He wipes the tears from his face, sucking in a breath as he looks around in slight confusion]

I'm sorry. I...I'm not...certain.

My Lady...where are you?

[Was this his White-Eyed Lady, come to collect him at last? Why didn't she know him?]

[He doesn't notice the PORTAL, but when he hears the voice he takes in a long shuttering breath, trying to control himself, but the panic is slow to die]

...I am dead.

[... Well, that's definitely not good. She doesn't look too surprised, though - she's fully aware that this city is capable of bringing back people from the dead. Her tone is still worried, although she tries to sound comforting.]

[Another voice he doesn't know. But he's getting used to them by now. He still has no idea that the book propped up next to the bed is his PORTAL, so he doesn't bother to actually look in that direction]

You sounded like you were in pain. Are you, y'know, okay now? You could probably just dream up some pain relief or whatever.

[A bitter, choked laughed. Almost a sob] I might prefer the pain at this point, to be honest.

Have you even woken up from a violent dream in which you died, and for those first few seconds you cannot quite let go of how real it had felt?

It was something like that, only...it's into the dream I have woken and my death was the reality.
