...It's from when they first met, and the Doctor thought Romanadvoratrelundar was far too long. Something like:

"How 'bout if I call you Romana?"

"I don't like Romana."

"It's either Romana or Fred."

"Alright then, call me Fred."

"Alright. Come along, Romana."

Which, er, doesn't make me get the joke being told here, but there you go.
