Do you use one of the layouts with LJ or do you have custom CSS?

LJ Layouts. But I suppose if I search the right spots, I could find custom CSS that has options to put a header in. . .Is it easier one way or the other?

I don't know about putting in a custom header with the LJ layouts. I use custom CSS, and most of the layouts you find (I like mintyapple - they have some good ones) have places to put the header. You just have to scroll through the coding to find it, but it's one of the only links in there, so it's not difficult to find.
