Program: PS CS2
Level: Medium
Translatable: No. Includes Selective Coloring.

Okay, the pic was taken from www.veer.com/

1. Crop your pic to 100x100 px. (Note that it looks good with almost any pic, as I found out by experimenting, but you  may not get the dark effect.) It does need a little sharpening, eh? So go to Filter>Sharpen>Smart Sharpen. Amount: 40% and Radius: 1,0.

2. Duplicate the base and set it to Screen, opacity 40%.

3. Make a new layer and fill it with #1e2756, nice mystical blue. Set the layer to Exclusion, opacity 100%.

4. Make a Selective Color layer ( Layer>New Adjustment Layer>Selective Color).Reds: -100, +40, 0, +18
Yellows: +100, +32, -100, 0
Greens: +100, 0, +100, +100
Cyans: +100, +100, 0, +45
Blues: +100, 0, 0, 0
Whites: +100, -40, 0, 0
Neutrals: +10, 0, 0, 0
Blacks: 0,0,0, +50

5. Color Balance layer.

Midtones: +26, -4, 0
Shadows: -20, 0, +11
Highlights: +15, +1, -24

6. New Adjustment Layer, Brightness/Contrast.B: +1C: +1

Done.
