There is one that fits your request perfectly but I have no title for it. It has House riding his motorcycle and getting chest pains and diagnosing himself. It has a good ending and the is a connection like you are looking for. Maybe someone else will recall the title.

There's an old collection of stories, the Denuo AU series, where Wilson (I think?) is an empath and there are various other paranormal elements. It's been a loooong time since I've read it, so my memory is a bit rusty. It's slash, but there's much more plot than porn, IIRC :)
