(( 22 year old Fiver is indeed wandering the castle, but he's not 'all there'. He hasn't been since he predicted major events of the wizarding war. Unfortunately he wasn't able to tell anyone about those events, because the visions nearly destroyed him. When he does talk, it's clear that he is in a sort of mental fog. ))
