I'd kind of prefer pre-series, but season 1 would do too. Dean is somehow brain-sucked by the monster of the week. The monster/hell-god/whatever feeds off of brain juice (which, in retrospect, is also similar to the baddie in "Sam Interrupted", but whatever) and is rendered very crazy. Not necessarily hallucinating, but unable to string his thoughts together, referencing things that make no sense, and being really afraid and child-like because his carefully constructed walls are all down (thanks to the monster).
John and/or Sam has to kill the baddie to make Dean normal again, but in the meantime they have to deal with crazy, babbling-nonsense Dean. And maybe Sam's in high school and is afraid his friends will see his fucked up big brother and think he's retarded or something.
