Some time after mid-season 6, Sam and Dean take out a nest of vampires. They rescue a civilian -- and it quickly becomes apparent that he/she was recently turned. Dean knows the signs.

They try for a cure. But whether the cure works or not depends on the civilian being honest about whether or not they've fed, or whether they can identify the right blood source, or if Sam and Dean can put it together in time, before they snap and attack one of them (probably Dean) . . .

To make a long story short, let's give Dean his own version of Heart.
