Yeah, Wilson finding out would only lead to pointless lectures, that wouldn't help at all.

Wilson definitely means well, but I think he isn't really able to truly grasp what it means to be in serious pain 24/7, and House is reluctant to open up to him about that, and this leads to pointless lectures sometimes. If House would be more forthcoming with information, things might be different, but House is too proud and/or too ashamed to really talk about his pain.

This scene was heavily on my mind when reading "Stitches". It makes perfect sense to inflict pain to distract oneself from another source of pain, and I could totally see House do this, however sad...
