So yeah. USUK, with preferably England as the mother. The pregnancy precedes as normal, but when he goes into labour, that's when everything goes wrong. I'd rather them not lose the baby, but have the carrier in extreme amounts of pain, passing out and not waking up, can't force the baby out properly etc. Recovery too, please. A slow recovery, with the 'father' being terrified at several points, thinking that the other won't make it.

As soon as I saw this I had an image in my head about it happening and couldn't stand to see this unfilled! May I proceed to begin writing? I don't think I'll make it incredibly long, but will be sure to add lots of angsty/comfort details.
