The doll that she's dangling is Pierrot, a clown from comedie de'l arte. He represents romantic, ideal love, although his love is always unrequited. I think the flapper is showing that she has a flippant personality and is uninterested in romance and love by dangling a little doll of him with as much care as she holds her cigarette. :)
