Genitalia aside, as a middle aged someone with ongoing age-related spine issues, and once a broken knee, I have often wondered the same - why are such basic things like rails and hand held shower heads not standard, and in the building code?

One thing I will note - standards are not always well thought out. Those ultra low toilet paper fixtures someone obviously thought were more handicapped accessible are, to me when my neck issues flare up, utterly inaccessible. Fortunately there are frequently spare rolls lying about.

Perinea aren't reachable by direct flow from the shower head, regardless of the sex of the washer. And fat people have to sash under bellies. Maybe if there were more women on design teams things would be better, but mostly it's just saving money, because the washer can wash without direct flow. You can't do away with the high head because there's no other way to wash hair. Adding anything beyond that adds maintenance and/or construction cost.
