Once one has peed there, it smells like the best place to pee. They'll all keep doing it til the smell is gone. Get to a decent pet store and get Nature's Miracle. It's an enzyme cleaner (another of that type shoudl work as well). DOUSE the area and let it soak into the grout. Then wipe it up and do it again. Then wash the floor. I'd douse it yet again a week later to be sore

Actually, I've been advised that it's n+1: three cats need four boxes. I was also advised to space the litter boxes out over the house rather than concentrating them in one room. This did the trick when one of my girls started peeing outside of the box ...
And yeah, definitely use an enzymatic odor remover -- the grout may have absorbed some odors. I've had great luck with Stink Free.
Vet check-ups sounds reasonable. A fairly cheap thing to try is putting double sided tape down where you don't want the cats to walk. Once you hit it with the enzyme solution (an absolute must to arrest the behavior) try putting carpet tape or some other double sided tape down. They will hate walking on it, and may avoid the area entirely. If they start peeing someplace else, then look for a second litter box location. Cats prefer a couple of spots.
Grendel used to do his business in bad places (my bed, for one) when he was sick, as his special way to let me know, so I join the chorus saying have them checked out at the vet. I've had success with pine cleaners so I've never tried the enzyme stuff but I bow to the greater collective wisdom.
Clean it a lot, then move their food bowls to the bathtub area. Yes, I do hate cats. I suppose you should move the litter boxes out of the bathroom to another lost area, how about the basement?
