Could this story be the one for #1. This doesn't fit all the criteria but it has some similar themes plus it is a good story. Your request just sound like something I read recently so far this is what I came up with. I'll keep looking on my lists though to see if I come up with another similar story.

I have no idea what #3 is, but I hope someone comes up with it. Somebody asked for a fic with pretty much that exact description months ago and it was never found.
