Well, I don't think that John and Sherlock are not equal - both are damaged and need each other, but John meeting Sherlock in other circumstances could work, you just have to define why John would then join Sherlock in his adventures.

You make a good point on how fandom views them. I'm looking from a more technical viewpoint. When they meet in canon, Sherlock has himself mostly together but John is in a bad place still recovering physically and mentally not well. While John complements Sherlock well, I don't think John sees himself as an equal partner and while he stands up for himself, maybe not as much as if he didn't feel somewhat dependent on Sherlock. As for how John gets involved in Sherlock's work, I have plans for that. ;)

I'm a fan of Arthur Conan Doyle's work - so I'm taking my conclusions from the books, Sherlock isn't stable when he meets John, it just takes a while for the reader to get it because we are seeing Sherlock from John's perspective.

Wellll, technically he's an MD, but no-one would ever say that, and a lot of doctors don't hold doctorates. It's very rarely that a PhD would refer/introduce themselves as 'Doctor' so the auto-assumption is medical. A few PhDs get intro'd as 'Dr', but it's normally a very formal title and someone of very high rank like Dr Rowan Williams, the archbishop of canterbury. You're more likely to say 'Jane Smith, PhD'.
