The forms for social security & passports are available online, and you should refer to them to make sure you bring all the identification asked for. I believe you'll need your DL/ID for Social Security.
If you find the ID Please resource unclear, calling TLC is a good way to get additional guidance. Finally, if you do bring everything you need and encounter difficulties at the SS office, do notify TLC as it's helpful in adapting their resources to what is actually happening on the ground. My experience with federal agencies has been positive, but YMMV.
You can change the gender marker on your passport with a simple letter from a medical Doc you don't have to be on hormones or have any surgery to have a male Passport. For social security they do require a letter stating you have had "irreversible" surgery or some other vague wording but there are ways around that too (which I will not post on an unlocked post in an online forum.
When I went to social security (march 2011) I filled out this form http://www.socialsecurity.gov/online/ss-5.pdf brought my name change court order, a letter from my surgeon, my old ss card, and ID to the social security office. I had no problems and received my new card in the mail after a few weeks.
