I live in southeastern PA. I was not asked for the original court order when changing any of my documents, but I did use certified copies (raised seal, etc.) when changing everything. In most instances you can get the certified document back, so that kept me from having to keep shelling out the money for more copies. Also, I too had a lot of problems at the DMV. Even with a court order to change my name and gender they wouldn't change the gender marker without having their *special and new* document filled out by a doctor. I can tell you, however, that you can have that document filled out by anyone (HRT doctor, therapist, GP, surgeon, etc.) So, if you don't want to wait for Dr. Garramone to send it back to you, you can have it filled out by someone else. I didn't have to change my name in PA, but as far as I can tell, there's just the one form to get your gender marker changed. It does have to be filled out by the doctor, but actually doesn't require surgery or even hormones. The provider just has to say that they treat gender change patients, that you are one of their patients, and that you need your marker changed.
It sounds like the DMV is harassing you. It doesn't always go without a hitch. I have a friend who was harassed at a DMV in NJ, the DMV worker showed him sex-change and name change guidelines from CANADA, and said that they were following those guidelines now. This was after him calling Trenton where the DMV headquarters are, and making sure he had all the required info. He finally threatened to call Garden State Equality and file a formal complaint and she changed his things. I suggest looking on Pennsylvania's DMV website, like the links others have posted, printing out the information, and just going to a different DMV.
I'm not sure if you got the first copy certified from your post, so I apologize if this is information you already have. You should be able to get the copy provided to you certified at the courthouse. The raised seal, etc. is often added by staff in a location other than the court room.
I agree with the suggestions to go to a different DMV if at all possible.
