Name change forms

I'm getting my name changed here in Alabama, and apparently I'm a real idiot when it comes to filling out forms, because there are some parts of the form I don't understand. They gave me three different papers. I understand the first one just fine; it's the common "What's your name, residence, etc?", but the other two make absolutely NO sense to me ( Read more... )

That's awesome :) I haven't been able to find any good docs for trans issues. My current one thinks the only reason trans people exist is because they get turned on by thinking of themselves as the opposite sex. ...so obviously, he's helpful with non trans issues though and I'm too afraid of looking elsewhere and finding someone even worse. I'd love to know about your doctors. Did you have any surgery done around here too, or just the name change? Because I don't know how I'd go about getting all that done (a hysterectomy first and foremost)

So you guys had no problems at all with the name change? A friend of mine told me that they wouldn't give you the name change unless you've been living at your current residence for a certain length of time, and since I've only been in this apartment for about two months, I'm not sure if it would work.

(Heh, sorry if I'm asking too many questions.)

Happy to give referrals. Sorry I don't get much computer time.

I've done everything to this point (therapy, T, and name change) in Alabama. When I made the decision to transition, I was in the middle of moving to Alabama, and I wanted to give myself 5-6 months to get settled here before starting (August 2005).

My therapist is very difficult to get a hold us, but she was wonderful to me. Annie M. Wells, Ph.D.

I left my "decree" form blank because I wasn't sure what to do with it. The nice lady at Probate helped me. Yeah, the other poster who said fill in the name stuff was right.

I know this is way late, but most credit unions and banks have notarys, so if you still havent found one, check your bank or credit union.

Another place is the libary. Bigger libaries usually have notarys.
